Persons needing birth or death certificates should
The Skokie Health Department has served as the local registrar for all births and deaths that have occurred within the Village since 1969. To receive certified copies, via U.S. mail the entitled person can do so by filling out the online application.

Applications
Birth Certificate
$10 for the first certified copy of a birth certificate and $5 for each additional copy at time of purchase. View Birth Certificate Application Form (PDF). Death Certificate

Payment
Check or money order for the fee payable to the Skokie Health Department. Visa, Mastercard and/or Discover credit card information if not using check or money order for payment. The Requester’s signature and relationship to the child or deceased / estate.Apply

Required Information
Required information for Birth Certificates if mailing without a printed form:- Child’s complete name as it appears on the certificate
- Child’s date of birth
- Child’s mother’s maiden name
- Child’s father’s name if it appears on the certificate
- Include a phone number
- Include a mailing address
- Include a legible, clear and large print photocopy of a valid drivers license or state ID of the person requesting the certificate
- Deceased name as it appears on the certificate
- Deceased date of death
- Include a phone number
- Include a mailing address
- Include a legible, clear and large print photocopy of a valid drivers license or state ID of the person requesting the certificate. *If Funeral Home, please also include the Funeral Directors License information
- Shipping and handling fees may be applied. Call the Health Department and ask for the rates
Time Allotment
Please allow five to seven business days. Call the Skokie Health Department and ask to speak to a registrar if you have any questions.
You can also visit https://www.cookcountyclerkil.gov/service/birth-certificates where certificates can be purchased from the Cook County Clerk’s Office. The Cook County Clerk has an office in Skokie at 5600 W. Old Orchard Road, Room 149 where birth and death certificates can be purchased in-person. You can reach the Skokie Cook County Clerk’s Office at 847-470-7233.
